Your Visit
Your initial visit will include a comprehensive diagnostic evaluation, which will be recorded in detail in your patient record. A copy of this will be sent to your referring physician and, if indicated, your physical therapist. If you do not wish your records to be released to anyone, please tell us at the time of your visit.
Information regarding your evaluation will not be released by our office to any one other than your referring physician or therapist without your written permission. Please remember to bring copies of any relevant imaging studies and reports, your insurance form or card and a list of your current medications to your appointment.
